How is it obtained? Is it a Wonder Card, like the TRU one? And can you nickname it?
First of all you have to complete the story mode of Pokemon Ranger. Doing this
will unlock Ranger Net. This is where you get four special missions to catch
rare Pokemon in this game. After beating the first three missions, go to the
Ranger Net menu and hold down the following buttons: R, X and left D-pad.
Doing so will unlock a password section. Here you must enter the password
P8M2-9D6F-43H7 (Mg35-Cpb8-4FW8 is the UK code). This will open up one final
mission. Now turn off your game
and restart it.
Go into the Ranger Net menu and you will see one last mission to complete
called "Recover the Precious Egg!"
After beating the mission the Manaphy Egg will be available in the Ranger
Net. In order to transfer it over to Diamond and Pearl you have to be able to
access a PokeMart. Now save your game, turn it off. Turn it on and go to the
main menu. Now get out your other DS and start up Pokemon Ranger. Go into
Ranger Net and there should be a button to start sending out the Manaphy Egg
through the Wireless function of the DS. Now Damond/Pearl should pick up
the signal. This means the egg has been sent and you now have the Legendary
Pokemon Manaphy, well at least it's egg form.

oops sorry sorry sorry I missunderstood
I don't know about the other two questions, but referring to the last question, yes, you can nickname it if you hatch its egg in Diamond or Pearl in your DS.
Get 2 DS's.
Put D/P in one, and Ranger in another. Turn on the Ranger game and go to the Ranger Net. Go to the Egg page, and find a button that says "Send to Diamond or Pearl."
Press it, and there will be a connection screen as it tries to connect to the nearest D/P game card. Start Diamond or Pearl, and go to the menu screen with "Continue," "New Game," "Wi-Fi Settings," and maybe "Mystery Gift."
However, this time, there will be a new button: Connect to Pokémon Ranger. Go to that and follow the instructions. This may take a few tries. It didn't work the first couple of times when I tried it.
