'k, so, if you don't see why I'm surprised at this, you obviously have never seen Bokusatsu Tenshi Dokuro-chan, possibly the most overly-bloody-to-the-point-it's-humorous anime I've ever seen.  It can't be beaten very easily either :P

So I have no clue what the plot is...I was discussing it with my friend and we came up with two scenarios: either you're Sakura and you have to avoid being decapitated or otherwise mutilated by Excalibolg, or you're Dokuro-chan and you get to smash everything in sight with absolutely no common sense involved.  That 18+ rating is kinda shaky too :P

So, any other surprising games for the PS2?
