If it's a game I don't know about, or a new series then I pick normal.
If it's a sequel or a prequel to a game I've played then I pick hard.
I pick normal then play through it on Hard. Only use easy when I'm trying to get something.
Usually Normal, but I've been starting new games on Hard or Very Hard recently.
largely depends on the game, but i'll go for the hardest mode that's available if possible, unless i know that the hardest mode is a complete cluster**** (mania in ToS, for example)
Normal, even if the game is really easy.
Then I go back for a challenge if I enjoyed the game.
Like how I just recently beat CoD4 on veteran for the achievements.
And HHM because its badass.

usually Normal first, then the hardest difficulty on my 2nd playthrough.
(Beat Halo 3 on legendary by myself, cod WaW. Half-way done cod 4)
Normal then do hard if its for the challenge or achievements. It always hurts me in cod games though because i start on normal and should start on hard.

If I ever decide to play the same game again I'll go for hard.

Normal, but Hard on games like Dynasty Warriors and Devil Kings.
Depending on my skill at the game, I'll pick the appropriate difficulty.
I'll play GH games on Expert, but I get raped at Left 4 Dead on Expert.
L4D will RAPE THE HELL OUT OF YOU on Expert. Even more so if you have more than one human players .-. because the CPU is better to me.
Normal at first. Once I beat the game I play it on a harder setting if it is worth more play through.
I usually just play on normal if it's an action or fighting game. For DDR though, I'll either play on Standard or Heavy, maybe challenge depending on the song.
Depends on the game. Usually the normal setting. Sometimes easy if I feel I might need to get used to the games workings.
