Nearly any first person shooter that's over T. I always know that "It's behind me, I know it is..." Then I die. The full reason why is because I think something is behind me in real life...creeping closer...closer...CLOSER!

The redeads from OoT used to scare the poop out of me when I first got the game. Other than that I haven't been scared by anything in a game.

Lol. I remember that. Anyway, my first shooter because I'd ALWAYS get killed from behind. I'd run away towards the exit, but the dang thing would always kill me from behind and made a horrible screeching sound. ;-; I can't tell you more, (game etc) because I was too young, all I remember is that horrible screech.
I think the only thing that's ever really made me jump in a game was the zombies in Conker's Bad Fur Day.
Even though they never surprise me anymore, they're still creepy as hell...especially the female ones.

When I was 5-6, I was scared of Banjo Kazooie :D
Not the character, just the game.
Only thing that actually scared me was in HL2, crawling through a small tunnel, then BAM zombie attack.

Also Metroid Prime 2 if you play it at like midnight with all the lights out and you hear the sound of the Metroids coming... You know the weird kinda shrieking noise they make...

Regenerators. You can get up close to the last one in the game without it attacking you. You know...the prison right before the last couple of rooms before Saddler.
Also, the Iron Maiden versions of them were worse. Those spikes were brutal, and those things would crawl around crazy fast if you knocked them down...which you usually had to.

I haven't even seen those yet. I stopped playing it and forgot about it, but Armadora is a ninny to beat.

I walk around for a while wondering where the hell the zombies are and the SECOND I cross a corner one jumps up on me.

THe FIRST time the piano in Mario 64 jumped out. That got me pretty good when I was six X_X;;
Chainsaw dude in RE4 for the first time. "Hm....I see something up ahead. What is that? Oh, its coming closer...wait, thats....OH FU**" (dies in one hit)
RE3: Nemesis, although I only played it for just one minute, the zombies looked scary, and it was scarier when they were in the PS1, because of the bad graphics.
The Regenerator in RE4 scared the hell out of me the first time. I open the door and BAM! Theres an ugly with sharp teeth. Other than that, when I was young, the Redeads from Zelda scared me a lot. I once even ran into my room because I heard a motorcycle in the distance and it sounded EXACTLY like the Redead screech.

Oh yea, I completely forgot The End's "angry speech" in MGS3 when you shoot his bird. Both scary and funny at the same time.

When I was little, I was playing Super Mario World for a LONG time, and it was about 9:30 (Which was pretty late for me when I was 7), and I was in the basement. I was running around in that darned Forest of Illusions (Which I couldn't figure out when I was little...), and I was just about to turn the game off, when my hand bumped on the cartrige, and screen went blank except for a glitched Mario, and the ghost house music played, really high pitched, and really loud. I almost frickin peed myself! 0_0
the redeads scared the hell out of me as a kid, and because of that I very rarely got the sun's song :P
uhh, the endless staircase in Mario 64, the pictures there scared me, as well as the piano.
the chozo ghosts in Metroid Prime scared me the first time through the game, as did the underwater tentacles (i have a fear of those tentacles/slimy things in general).
the monsters in banjo-kazooie & tooie that would come out of the walls. scared me every time, no matter how many times I'd go there. also the mummies.
RE4 wasn't that scary except for the regenerators (ran past all of them the first time through, didn't even get the infrared scope).
HL2 scared me more than anything I've played. The headcrabs were terrifying O_O
funny thing is, Metroids never scared me. just the headcrabs did.

I would've laughed if you didn't put "gamecube," because it was impossible for anything in the original to be scary. Nothing looked or sounded scary at all. Horrible, but not scary.

Hence I said kind of freaky. I haven't ever really played a game that has honestly scared me.
Mario is Missing
