I saw an article about a man who won a million dollars in a Puerto Rican casino and when the cashier came over, she said, "Oh, this machine must be malfunctioning." and unplugged it. He got nothing. I personally think they used the "It's malfunctioning" excuse just so they didn't have to pay him.
