I jumped through the rings in the correct order twice, (Once before Truth is killed, and once after) and the skull wasn't there. Does it have be on Legendary, or does Campaign Scoring have to be on or something?

You have to do it after Truth is killed, although I don't know exactly if it would harm it to do it before.
