For me, it was when I fell off a treadmill. (Yes, I had the clip on too, but the intercourse ing thing just kept going.)
The worse part about it, was there was a wall behind me. I got stuck, as my legs were being torn apart by the conveier.
My legs are still recovering, and it was 2 years ago!

This girl trough a brick at my face when I was 3 in Israel.
I went down an inflatable slide and jumped off when I was at the bottom and hit my head. That happened a few weeks ago.
passed out (for unkown reasons) and destroyed two teeth, while injuring my jaw as well. Having braces on afterwards isn't fun...instead of the usual afternoon of recovery per wire change, it takes about a week X_X;;
When I was a kid, I got something--I think it was a acorn--stuck in my throat. I had to go to the hospital and get it removed. However, I've never really been injured. There was that one incident at the end of 9th grade where my shoulder got kicked and wouldn't work right for a whole month, though.
I was in a rocker when I was four, and I fell. My chin apparently split open, and I had to get stiches. It doesn't make sense! I fell backwards, the chair with me, my chin hit nothing, and the back of my head was sore. >_>

I think when I broke my arm when I was 8.
Now I can't straighten it out or write with my left hand (I was left handed).
i think it was 5 years ago, my sister broke my left colar bone
I was riding my bike down a hill at, what, probably 20 or 25 mph, and my tire hit a rock. I lost balance, tumbled halfway down the hill, then tumbled in the woods into a thorn bush. I had two big scrape marks on my back, one on my knee, one on both of my shins, a small one and a big one on my left shoulder, and a big one on my stomach. And then, on top of that, I had thorns all over my body, and on top of THAT, I had a broken AND dislocated collar-bone (Same one.) and a spranged wrist.
I've broke/sprained the same finger 4 times.
Let's see... 7th grade, I was playing our last soccer game. I was going for a goal when some stupid chick pushed me, causing my foot to bend forwards too much as I fell. I ended up with fractures and torn ligaments.
Hematoma on my leg, broke my wrist, broke my right arm twice and craked my left once, and umm... I can't think of any more now, but I probably will later :P
Oh, wait. Does teeth count? Because when I was little, my teeth were so frigging messed up :D Once I woke up in the morning and my right cheek was swelled like 7 inches out XD So I went to the dentist and I had an abscessed tooth, godarn that thing hurt like hell D:
Well, I don't really get hurt that much. But last summer I fell on something sharp at night and cut me knee open. Had to get 5 stitches. I just have a scar now.
Haven't broken any bones....
But I have broken two of my teeth!
First one I hit with a skee ball (Don't ask, I don't really know myself) and had to get a route canal. Boy did that suck.
The second one broke in two, still connected to my gums, when I ate a piece of cauliflower. Yeah, that's right, CAULIFLOWER.
Oh, and I also have a scar on my forehead. I tripped and hit my head on the corner of a wooden door.
I was riding my bike on the sidewalk for some reason. I hit a mailbox and broke my arm

When I was sleeping at my grandpa's house I did not move my right leg with out noticeing when I was 6 and It was hurting and very stiff, Now days my knee caps hurts.

Not my worst injury, but my most exciting one: We have a lot of Coyotes in our backyard, since we practically live at the edge of a forest by a lake. I was out with my dog, and suddenly she began barking, growling, and backing away. I had no idea what she was doing, until I suddenly heard something moving around in the bushes. It was really early in the morning (about 9 AM) and so I could see something brown. At first I thought it was a bunny, so I got closer to scare it away. But next second, a Coyote jumped out at me!
I got freaked out, so I bent back (I'd like to say Matrix-style, but it probably didn't look anything like that) and the Coyote had jumped forward and raked at my right shoulder with its claws. I fell back as it pushed into me, and then it ran off. By then my dog had ran home (>_> Man's best friend my ass. I still love that dog, though XD.) so the Coyote didn't get her. I'm just glad the Coyote didn't feel like eating me for lunch, I didn't have anything to defend myself with, and although I thought I could put up a fight, I knew I wouldn't win.
Anyway, I let the wound heal on its own, (that's how I always solve any injuries I have) and I still have a line across my shoulder where the Coyote clawed me.
Other injuries would include: Twisted ankle from soccer. Also, I once, as a little kid, pulled a giant mirror toward me that fell and crashed over my head. Surprisingly, nothing at all happened. No scars, scratches, stitches, bruises, or damage in any way, shape, or form. I don't even remember the incident. I've seen pictures of the shattered mirror, and I've been told about it. Lastly, I once was jumping off a swing, got my shirt stuck on it, and fell back and lost my breath. I only got that darn breath back right as I was about to pass out from not being able to breath. That was a scary memory, I thought I was going to die from lack of oxygen XD.
4 things I can remember. No, wait, 5!
1. Falling on a concrete floor (I was really young)
2. Falling on a hardwood floor (some b***** in Kindergarten did this. Ain't here no more, the idiot.)
3. Watching a Dodgeball game between adults, one had a misthrow, nailed me in the side of the head while I was talking to a friend, then due to the recoil, my head hit a concrete wall (my head seems to like concrete...).
4. Some younger kid attempting to pick me up thinking he was soooooooo tough, and accidentally dropping me... right into a metal door.
5. I was riding my bike, went over a lip, wheels slipped in the dirt, fell and skidded BOTH of my knes along the ground, with large gashes.
Yeah, for some reason, my brain hasn't been performing deficiently, either, seeing how smart I am.

Erm, that's why if you live there, OWN A GUN. Besides, if a coyote was crazy enough to attack a human, especially a human with a dog it don't know, it's obviously sick, and could use a euthanasia.

you've had experiences with cayots as well? You should hear the story about my dad and a cayotoe.
BTW, if it attacked without being provoced it's probably sick or something....should've gotten that checked out X_X;;
I got punched right in the jaw and the lower part of my mouth a bit...
Since I was wearing braces, the braces cut up my lips and both top and bottom all around needed stitches.
The braces kept my teeth in my mouth though... 100% chance of 2 or more being knocked out if it wasn't for my braces... I was within 1 month of having them removed forever... and this was a few years ago.. guess what.. I still have braces...
5 of my teeth were loosened, 2 of which went back about half an inch and were angled all funnily.. instead of facing up.. there were angled 45* back towards me... so.. they had to be pulled back into place....
Worst physical injury I guess.. I don't think I've ever bled so much before.
Yay for random 4-vs-1, unprovoked muggings in the daylight... they stole my DS which was hidden in my pocket too.. woo...
I couldn't even kiss my g/f at the time any more.. that was the worst part about it.. worse than having braces for an extra 2+ years...
In 1st grade, some kids pushed me off a ledge on the playground, then I fell and hurt my leg.
I don't get hurt much. Never broken a bone or anything. The worst injury I can think of was in the first grade. I was sitting at a table outside talking to a friend, while some people nearby were playing football. The ball randomly hit my head. I think that's the worst I can remember. I live a sheltered life.
When I was eleven, I was cooking pasta or something. When I was draining the cooking water, some of it splashed on my hand. I recoiled and dropped the pot onto myself, and the water fell on my stomach and foot. Wherever the water directly touched my skin, the skin melted there. I suffered 3rd degree burns on my stomach and 2nd degree burns on my foot.
When I was ten, I had the back of my hand sliced open on a truck. I think earlier that day, I fell about three stories. I would have fallen on my back, but I managed to turn around in midair, so I ended up landing on my hands and feet. Unfortunately, my knees have been a little stiff and there's mild rubbing in the bones, though not enough to hinder me at the moment.
I think when I was about seven that I was in a field playing on a moving tractor by running on the wheel and keeping up. After a while I got tired and I feel off. The tractor turned around and ran over me, so I suffered bruises to the abdomen and groin. There's a chance that may have resulted in me being sterile, but I haven't tested to find out.
I had to get rods and extra bone in my back to correct scoliosis. 3-month total recovery time. But oh, no. That wasn't enough. Turns out that I was allergic to the nickel in the stainless steel. So then my body tried to reject the rods and caused inflammation and pussing, and I had to get a second surgery to get the rods excised.

Man, that's pretty bad. How did you manage to fall down a 3-story building? Though it sounds like fun, if I managed to land on my hands and feet like you did.
In Third Grade, I was on my school's basketball team (only played that one year) and it was practice. I was sitting on bleachers that were closed (meaning, only the very top part was open). I was sidelined for the moment, but then the coach asked me to come in to play.
So, I got up, my foot slipped, and I did a faceplant on the gym floor. >_> Chipped one of my front teeth into three pieces. I had to go to the dentist and get a fake part for the bottom of my tooth. Since then, I've gotten that part replaced about three times.
I got hit by a motorcycle and passed out. When I woke up my arm was covered in blood and I broke it in two places.
I cut my arms a lot, but other than that not really anything.
