I don't think I've ever seen a game with presentation this poor. They should have just made it live-action, the system it was on could have handled it.
Man, now I regret buying Red Dead Redemption. :P
But yeah, worst game you've played? Mine would probably be Superman 64. It barely works. Either that or Final Fantasy XIII with its unnecessary padding and boring battles.

The company was so desperate for games that they pretty much let anyone make them for it.

I generally don't get a ton of video game funds, so I read lots of reviews and rarely get into new IPs unless the genre is appealing (example: I got into inFAMOUS, but only because it's a superhero, electricity is my favorite power, and the game got a 9). As such, I don't actually run into genuinely bad games that often, unless a series I follow takes a sudden dive.
Which brings me to my point: the worst game I've played, at least as far as I can remember (I know for a fact my taste was less discerning as a child, but my memory fails me on specifics), would probably have to be Spider-man 3. It's ugly, it's poorly controlled, New York looks pathetic for a PS3 game, it takes nearly everything fun about 2 and throws it out the window, combat is bad, it's just plain awful. I've had it for two or three years now, and I still don't have the will to finish it.
I played Spiderman 3 for about thirty minutes, and was done.
I was really disappointed in Wolverine's Adamantium Rage for the SNES. I thought it was pretty bad.
Hard to consider anything the "worst game ever", what with games coming out based on kids TV shows and the like.

For the Wii, yes that did suck.
360/PS3 wise it was a good game.

To me the PS360 version was an improvement to the Wii/PS2.
The worst game that I've ever played is probably Master Chu and the Drunkard Hu, on the NES.
