Well, it's a good thing you have approval setup.
I know constant deletion of the accounts and threads can become a pain in the rear.

Turn e-mail confirmation on. Spambots can't get past that, can they?

Spambots can get past that pretty easily anymore. Super strong captchas make it way harder for legitimate users to sign up. Etc.

There's not a ton of options right now.

What about moderately strong CAPTCHA's?  Software that could read images like humans can probably isn't available to the average spambot.

A CAPTCHA is a CAPTCHA.
If it can easily be bypassed by a human it can be easily bypassed by a bot.

Like I said before, bots are becoming more intelligent these days.

I saw this somewhere... but I don't remember where.

The captcha gives you a picture with a bunch of text but tells you to "only copy the quoted text/red text/underlined text" ..which is something a bot wouldn't be able to do. (it might copy all the text it sees)
